The Memory Care Support Program is a specialized care program that focuses on in-home safety and improving quality of life for those with Alzheimer’s Disease, dementia, and other forms of cognitive impairment.

The Memory Care Support Program offers:

  • Dementia Care Certification for caregivers taught by local expert, Tiffany Mikles, owner of Dementia Care Coaching
  • In-home Safety Management to provide peace of mind
  • Daily focus on client safety and comfort
  • Individualized care plan development
  • Interventions for all stages of dementia
  • Communication techniques to minimize client and family frustration
  • Prevention strategies for wandering, resistant, combative, and Sundowning behaviors
  • Accompany to social events and activities in your community

A specialized care program that focuses on chronic disease management for clients diagnosed with Congestive Heart Failure, Heart Attack, or Pneumonia. This program can also be effective for other chronic diseases.

H.O.M.E. Program

Guidance’s H.O.M.E. (Health Overview Management & Enrichment) Program helps with chronic disease management for:

  • Congestive Heart Failure (CHF)
  • Heart Attack (Acute Myocardial Infarction or AMI)
  • Pneumonia

The H.O.M.E Program focuses on reducing recovery complications and PREVENTABLE trips to the ER by:

  • Developing personalized care plans with disease-specific health goals
  • Increasing client compliance with doctor’s recommendations at home
  • Encouraging lifestyle modifications to help manage chronic disease
  • Assisting with medication compliance through medication reminders
  • Improving quality of life through social enrichment
  • Communicating and coordinating care with your doctor and your entire healthcare team

Over time, this can be very challenging to maintain without receiving any relief or support, and the family caregiver’s health can suffer from the excessive stress and responsibility of caring for their loved ones. Respite care focuses on helping you create time to take care of yourself.

Veterans, their spouses, or surviving spouses may qualify for a special disability pension benefit that can be used to cover the costs of home care services.

To be eligible for Veterans Aid and Attendance, the veteran must:

  • Be 65 years old or if younger, be unemployable
  • Have served at least 90 days of active duty, one day of which was during a period of war
  • Meet income and asset requirements

The veteran or spouse must require assistance to eat, dress and undress, manage personal care, or have other medically necessary care needs.  Benefits are paid monthly, directly to the veteran or surviving spouse.

Veteran’s Aid and Attendance monthly pension amounts vary based on need and financial qualifications. Maximum benefits are listed below:

  • Two veteran spouses                                          $2,789
  • Married veteran needing care                            $2,085
  • Married veteran, only spouse needs care        $1,130
  • Single veteran                                                       $1,758
  • Surviving spouse                                                  $1,130
